We understand your child needs daily medical care and monitoring. SSM Cardinal Glennon helps ease the process by collaborating with all your doctors, specialists and other providers to ensure your child gets the care he or she needs. In addition, we also support you with answers and information so you understand the process.
To help, we are introducing the Outpatient Mobile App.
What is Outpatient?
Outpatient is a care coordination app that lets patient families communicate directly with Nurse Coordinators and Case Managers about your child and the care plan.
It’s a simple, easy-to-use App (downloaded on your phone) so you can track scheduled activities, medications and appointments. Plus, you can receive and share updates, and ask any questions along the way. In addition to your SSM care team, you can invite any close family members that you want to follow along with you.
